Crye style Camelbak pouches.
Specific improvements that I added to Crye's design:
Wider 50mm webbing at the base
Folded cordura over bottom edge, which is more durable, in lieu of binding tape.

Light weight low profile 3 row belt

2.5" wide, utilising 3/4" Multicam webbing to reduce the footprint

Multicam Omega mesh laminated on spacer mesh for weight saving

British Milspec V twill binding (stiffer and more durable than the printed MC binding) for a more rigid structure

25mm cobra buckle

If I were to make more in the future, I would go for a dual adjust cobra buckle.

Wolf grey cobra belt
1mm HDPE stiffener
Titan blue cobra buckle is a good colour match for wolf grey, but shame that it's shiny.

Garmin Foretex 401 Flip down case

The GPS would be orientated vertically, so that the footprint of the pouch is minimised to one column of PALS.

Medic Inserts for LBT blow out pouch

These inserts are made of a stretch fabric so that you can stuff more into them.

Have you thought of using a zipper rather than velcro? Especially with stretch fabric, stuffed full I could see the velcro opening just from tension, not to mention the much smaller profile a zipper would have.

1" velcro is more than sufficient to keep everything in, see below for customer photos: he's got a celox, FFD, chest seals and nasopharyngeal airways (and he says that he can stuff a bit more into it). Also velcro makes it easier to open when under stress/reduced dexterity.
