I prefer custom-made medical insert, similar to ATS/OSOE, but slightly improved by my own. I like its versatility. I can put it in to my chest rig for green side ops, to plate carrier for urban ops or put it into backpack to climbing or another outdoor activities. Also like it because is smaller than another pouches and fits to pouch only 2 PALS wide. I'm smaller physique so I don't usually have 3 PALS wide left for medic pouch.

Diamonback tactical Small medical releasable pouch. I have had the current issued IFAK from the USMC and this one and its pretty much night and day when compared together. I like the fact that I can now mount my meds on the back yet still have the ability to get to them just by pulling the release cable. That way I can mount mags, pistol, radio, and pyro up front. And the pouch also has molle tabs so if you want to hard mount it you can. Also the inside is similar to one like the EI med pouch design with elastic to hold items all over. I might get a Medium one next time.

I am using LBT-0250B. It is large enough to take everything you need in average IFAK and still has space for E&E kit and other stuff. Its advantage is you can hang it under your 1st line or pants belt so you have spare space for another pouches. However you can't tear it away quickly so it's better to make an insert for it.

Its low profile, holds everything I need as well as it mounts directly and securly to a MOLLE belt as well as my regular trouser belt. With regards to ease of access I have begun to really "slick down" the front and sides of my load out and I have found this pouch to be easy to access with either hand even when mounted center back on my belt line which is were I have been stowing my med kit.. Now I need to find one in AOR1..

Gotta give my vote to the ATS Small Medical Pouch.

Has enough room for my IFAK content, and does not take up a lot of space on the rig. Tear-Away is a must.

Just got a Tyr Tactical Small IFAK Medical Pouch, Cutaway. So far its looking good, gonna test it more on exercise soon.

I m using a SOE TEar Off IFAK for some years, pretty solid and fits everything I need.

Got on in OD for civil use and one in German Flecktarn Camo...
